Creating and Sharing Intake Forms

Intake forms are your online front door. Prospective students and parents fill them in to express interest in lessons, and their submissions flow into Clefora as leads for you to review.

  1. Open the Hub and go to Intake forms.
  2. Click Create form.
  3. Enter a form title (e.g. “Piano Lessons Enquiry”).
  4. Add fields by clicking Add field. For each field, choose:
    • Field type — Text, email, phone, dropdown, multi-select, or long text.
    • Label — The question or prompt (e.g. “Student’s first name”).
    • Required — Whether the field must be filled in to submit.
  5. Drag fields to reorder them.
  6. Click Save.

Once saved, your form has a unique shareable link. To get it:

  1. Open the form from Intake forms.
  2. Click Share.
  3. Copy the link and share it via email, social media, or messaging apps.

You can embed your intake form on your website in two ways:

  • Direct link — Add the form URL as a link or button on your site. Visitors click through to the hosted form.
  • Iframe embed — Copy the embed code from the Share dialog and paste it into your website’s HTML to display the form inline.

To update a form after creating it:

  1. Go to Intake forms and click the form title.
  2. Make your changes (add, remove, or reorder fields).
  3. Click Save.
